Copying everything from CD HOSP folder to hard drive works on Vista 64. To get the saving to work, either modify the hospital.cfg file so the entry:

SAVE_OVERRIDE=c:\hosp\

contains the correct root folder of where you copied the hosp file into, or just make sure you copy the HOSP folder into c:\hosp

Don't know if it was required, but also set the compatibility mode on the winmain.exe file to windows xp.
