That thing is a "customer", that eats people, has a large organ that's totally not a scrotum that must be destroyed first before being damaged and can open doors with its innards upon being defeated.

Then suddenly, a Gieger-esque lvel! Is this Contra? Nope, better! Also, plot is that an uncontrolled bio-weapon turns an entire city into a fleshy horror landscape, with the Butcher trapped inside. More meat for him!
The level is called "Moving Bowels". Yep.
The final boss?
Nope. Suddenly a giant, horryfing dinosaur thing shows up, but I cannot capture the pic correctly. After a harsh battle, the Butcher merges with the Demiurge (the bioweapon's name), mutates and becomes an endless mass of flesh that covers the planet, eating everything, rendering Earth into a pink blob drifting in space, a place where conflict is inexistent and peace is eternal.
Kinda like the Evangelion movie thing but with no take-backs.
Now, for something different, my hugely modded playthrough of XCOM2!!
The mod was so inane it broke my game.
With that name, surely he was an alien clone.
Hardest mission so far. My guys were spent by then. Using larger pods to compensate for a larger starting squad sometimes backfires. Also, of course the game followed by a "Base Defense" mission while half of my team was wounded.
