my predictions:
* regenerating health
* 12-16 skills
* respawn on death
* staves become even more gun-like and have auto-aim
* same retarded voice actors from oblivion
* mudcrabs

They will go with the same province because everything else isn't generic.

Your ignore list.
**Level scalling 2.0:

- Monters level = Player level - 5;
- Loot Level = Player level + 10;

**Quest automatic solving based on character statistics and dice rolls (because bethesda wants to please hardcore rpg players). There will be quest items/spells that increase the probability of solving the quest;

**The Secret Mudcrab Level accessible by combining a leg and a spell scroll at the alchemy screen;

Now with even more thugs wearing enchanted daedric gear in the endgame!!

Meet hundreds of characters that always have something new to say in 3 variations.

Got stuck in the game? Go to the deadric shrine and donate 1000 Gold pieces and your quest journal becomes automatically cleared!

All the stealth skills will be combined into "Thievery", all the spell colleges will be combined into "Magic" and all weapon skills will be combined into "Fighting".

Bows will always hit because we've test-played with them and understood that missing with a bow is not fun.

If Fallout 3 tanks and Todd and Pete are fired, we might have a slight chance at a decent TES game again.

Like that'll happen.

If Fallout 3 sells a billion copies and Todd and Pete are promoted, the next TES game we'll see will be an MMORPG.

That's pretty much guaranteed to happen.

*Bullet time mode
*No player death
*Instead of dialog, we'll play Galaxian and if we get a really high score, we get useful information. Also, if you kill somebody, you play another mini-game to see if you can say IT WASN'T ME, though everyone will forget in 24 hours, of course.
*When traveling, you'll play a mini-game with a dance mat to see how fast you walk.
*If you get diseased, you have to rest an hour.
*Loot will scale even in your inventory, so you'll never have to collect new stuff.

That would actually be a plausible design choice for publishers. They'd only need to tie it to a skill, so higher the skill, further scaling in inventory. Next-gen kids would love that.

I'm off topic, fully aware of the fact that you're referring to VATS and possibly offending the hivemind, but implemented right, I believe that an RPG with Max Payne style bullet time mode would be pretty damn cool.

On Topic: The ability to change race, class and starsign (which have been molded together into FIGHTER, MAGE, THIEF and EVERYTHIN') is readily available from the character screen.
