CRPGs always make penis sizes decrease.
1) You are wasting a lot of time being sedentary, and that means no blood flow in your genital areas and the weakening of your pelvic muscles
2) The lack of excersice also lowers testosterone and bloodlow and thus penis size
3) Your social skills deteoriate and you are getting socially isolated meaning less opportunities for sex and thus less training of your penis muscles and thus enlarging them
4) The sedentary lifestyle plus the depression due to social isolation leads you to gaining weight, and fat both slows down blood circulation and creates fatty tissue around your penis, making it smaller overall.

I seem to recall that it shrunk a bit when I decided to unlock the final 2 achievements in Dragon Age by doing the Alistair & Zevran romances.
