What's the name of the CRT shader? Something fucked going on at the top and bottom of screen.

"CRT-geom". I tweaked my settings a bit and now it looks fine.
I'm not really experienced with setting up the shaders/filters in RetroArch, so expect horrible results while I'm playing around.

IIRC there are level streaming issues. Personally, I had horrid stuttering even at 60 fps, and no matter how much I googled the issue, no one seems to encounter it (or have a possible solution). I tried it on 2 different rigs with same results.
