Steam is shit at detecting playtime. Also doesn't detect most mods (that change the .exe or use a launcher) unless you manually add them, even then they just sometimes refuse to work with Sturm overlay. My real "most played" is probably modded Skyrim with like 600-1k hrs, and probably some emulated games coming in at 50-100hrs or so.

I barely use Steam. lol I still remember uninstalling Still Life though soon as I encountered a faggot slide puzzle. Never did finish Divinity either and had already beaten Hitman a couple times before Steam came along.

SKSE definitely didn't count when I was using it. When I fired it up I was still shown as "online" and not in-game, plus the "Time played" didn't change.

I don't think all mods+fan missions avaliable hit that much. I guess that new Revision (incoming in next week) will add another +100h on that you need to surpass Skyrimjob somehow, it's your duty as a man.

Full disclosure:
Ut2k4 is only since I had steam. Would prolly be a few thousand more hours if the pre-steam days counted.
Out of the 100hrs of magicka, probably 30 were bug related not-playing sessions.
E.Y.E is love, E.Y.E is life.
I only ever played a pirated eu4, else it would easily top this entire list. Same for vicky 2, of which I played many more hours, but in pirated form since I don't have the expansions.
